Our Mission

The mission of the Celebrate Life Foundation is to Celebrate the Life of hospice patients with life limiting illnesses and support the families that surround them. The Foundation is a non-profit organization which is funded by the generous donations from families and friends of hospice patients.

Ways to Donate

There are several ways to donate to the Celebrate Life! Foundation. You can make a donation online, use the remittance envelope found in the patient and family handbook for hospice, or print and mail a donation form to: 110 Polaris Parkway, Suite 302, Westerville, OH 43082.
